We are scheduling appointments for new and established patients.  Contact us now by calling 770-405-0391.  We also encourage you to email us at appointments@scottgynecology.com.  In your email just state your name, phone number and when you would like an appointment to see Dr. Scott.  We will promptly call you back to schedule your appointment. 

You may click here to access a medical record release form so that we can receive your medical record before your first visit.  We also ask that you print and fill out a medical history form.  You may fax your release form to your previous provider or bring with you for your visit.  Please make sure you have the following when you visit us:

• Picture ID
• Copayment
